I would suggest "Identify the deliverables" at the 9th resolution.

Complications come from the lack of understanding of the 'thing', either agency side or between the client and the agency, hence clarifying the deliverables and their acceptance criteria upfront can be a real time saver and enables to address gaps and misunderstanding before spending too much time on the wrong direction :)

Great post.

On point #3, which I agree 100% on, do you use any particular software or web tools to generate the high level milestone graphs? I usually create mine by hand for my team, but I think its an essential part of the process especially when you have different stakeholders involved and they need to see the larger perspective to buy into the details.

But I'd love a tool to do create those for me, and allow me to shift them, and well as drill down. That'd make my life easier!

Point #1 is the single most violated cornerstone of meetings, not just project management. An agenda is your flight plan. Without it there's no way to steer the conversation because you don't know where you're going.

For point #5, Review project status frequently with stakeholders. It is important to define what frequently is. I think the project manager should do this once a month, and no more (not once a week).

If there are any problems then he should communicate these problems early (but the project sponsor first).
